WE WANT A BABY:For about thirty years since the moment the first child was born "from a test-tube", public attention has been paid to IVP (In vitro production). The basis of these technologies for giving life is IVF/ET (In vitro fertilization & embryo transfer).
A child doesn`t come "from a test-tube". The first crucial stages, the fertilization and the beginning of development, take place in a special incubator.
The IVF method enables one to take a number female reproductive cells, fertilize them by spermatozoids outside the body and after a few days of cultivation to transfer them into the uterus of the patient. With this treatment the probability of pregnancy is 40% or more.
Today difficulties in conceiving a child affect one in 4 couples, half of these cases are due to a disorder in the female reproductive system, but men are almost "catching up" with women. The quality of sperm has been more than halved in the last 50 years.

Briefly about the main stages of the IVF method ("Test-tube children")EXAMINATION OF PATIENTS
It`s a very important stage and it is best to have this carried out at our clinic. It helps to make a detailed assessment of the features of the reproductive system of both spouses, maximizing the chances of an effective treatment by the IVF method that will lead to the desired pregnancy.
INDUCTION OF OVULATION Each month only one oocyte matures in a woman. The probability of pregnancy in a healthy couple in a natural cycle is on average 25%. We emphasize ' healthy'. Patients undergoing treatment by the IVF method take special medicines, which help to produce 5-10 times more oocytes. Thus the probability of pregnancy increases by 1.5 - 2 times.
OOCYTE RETRIEVAL This is carried out under general anaesthesia under ultrasound control for 5-7 minutes. Manipulation takes place as quickly and painlessly as possible for the patient. After the procedure, the woman stays for 1.5-2 hours under observation and is then usually discharged.
FERTILIZATION AND CULTIVATION Special incubators ensure optimum conditions for successful cultivation (growing) of human cells outside the body. In the incubator the "meeting" takes place of oocyte with spermatozoids, and further development of obtained fetuses take place. All manipulations with them are carried out quickly but with utmost care. The cultivation stage may last 48-120 hours.
TRANSFER OF EMBRYOS INTO UTERUS Usually it`s an easy manipulation in which the best of the obtained embryos are introduced into the uterus cavity. The woman stays under observation for several hours. She needs to calm down and relax. Her mood will play a crucial role in determining whether her body accepts new life or not.
Spare embryos of good quality can be frozen for an unlimited time. In the future if the present attempt still fails to provide good results, embryos can be unfrozen and transferred into the uterus months or even years later.
SUPPORT OF THE SECOND PHASE OF THE CYCLE The woman must spend the next 2-2.5 weeks in a condition of physical, emotional and sexual rest. She has to follow the doctor's requirements for the support of endometrium, taking progesterone, estrogen and other medicines. Even if there are changes in her general state of health or if there are signs of unusual symptoms, she must not stop taking the medicines without consulting a doctor.
DIAGNOSTICS OF PREGNANCY BIOCHEMICAL PREGNANCY tests will be carried out at a time set by the doctor. The most informative method is to determine the levels of chorion gonadotropic hormone (CGH) in the blood and later the determination of a fertile ovum by ultrasound (approximately 5 weeks after transfer) can be carried out. Premature application of the often unreliable method of urine testing may both disappoint and cause much distress.
Results of treatment in our clinic
The reproductive system of a human being is too complex to be compared with the repairing of even the most complex equipment. The probability of success with any one couple can only be considered in the light of a range of factors surrounding their health, both physical and mental, and their personal and public lives.
In an average year, 40% of our patients manage to conceive. And that happens in families where conceiving was almost or completely impossible. Some of them have results from their first attempt - others from their second or third. Some were previously treated in other clinics. We can record only our own current statistics. The probability of pregnancy from three attempts is above 90 percent.
Each couple has their own probability figure, determined by dozens of varying factors including some inaccessible to analysis. Among them we specify - the age of the woman, the duration of the sterility period and the moral and psychological climate in the family.

Methods of assisted reproductionIVF, ICSI, IUI (intra-uterus insemination)
- ways of overcoming sterility. These technologies, previously named as artificial fertilization, should in reality be differently named. Their bases are the reproduction of natural processes and even a minute deviation from a natural plan in such a complex process can lead to failure.
IVF / ET - in vitro fertilization & embryo transfer.
The "Test-tube children" program enables us to have descendants in spite of the most difficult forms of sterility. The obtained embryos are cultivated for several days and then are transferred into the patient's uterus. The chances of giving birth to a healthy child from the first attempt even with the most "hopeless" forms of sterility, are, at present, 35-45%. Usually we recommend planning 3-4 attempts. With such an approach the cumulative effect reaches 90%.
ICSI - Intracytoplasmic Sperm Injection
- injection of spermatozoid into the cytoplasm of the oocyte is a type of the IVF method. It is used for couples with lowered indexes of mobility and general maintenance of spermatozoids. If conceiving by the classical IVF method is impossible, micro-manipulation is carried out where single sperm are induced into each mature oocyte obtained.
Sometimes fertilization doesn`t happen despite the fact that the spermatozoids and the oocyte are both normal. Such difficulties appear to be due to structural features of the external oocyte membrane when it becomes impermeable for spermatozoids. The ICSI solves this problem as well.
IUI - intra-uterus insemination
is a method very close to natural fertilization. It offers the introduction of the processed sperm into the cervix uteri or the uterus. The necessary conditions for carrying out this method are: passable uterine ducts and favorable indexes of husband`s spermogram. Unfortunately the effectiveness of this method is only about 15 % worldwide. With the absence of pregnancy it is necessary to refer to the IVF/ET method.
The program of insemination by donor`s sperm
allows us to solve the problem of sterility in families with a complete lack of husband`s spermatozoids, with several genetic disorders or for single women. The effectiveness of this method is much higher according to our observations than by the IUI of a husband`s sperm and reaches 20-25% due to a high fertilization ability of the donor sperm.
It should be noted that the decision to carry out ICSI procedure can be
made only after a detailed consultation with a doctor- geneticist and
after receiving the results of special examination - screening for men.
Screening includes receiving chromosome set (caryotype) and molecular
observation of Y-chromosome at DNA level. It is also necessary to exclude the bearing of several spread mutations leading to mucoviscidosis. All these measures are intended to prevent the transfer of hereditary negative information and to make the correct selection of methods of assisted reproduction. The complete results of this examination are usually ready 7-8 weeks after drawing samples of the patient`s blood.
Specialists and experts were very much worried about possible negative consequences of ICSI application. However, at the moment, with skilled organization of work, the feared increase in the level of genetic disorders and other problems has not been observed.
The program of DONATION OF OOCYTES differs from the conventional IVF method by the usage of the oocytes of women donors who have high reproductive idexes. Donor oocytes, fertilized by the spermatozoids of the husband are carried into the wife`s uterus cavity. With a positive outcome the wife then becomes the social and juridical mother of a long-desired child borne by her.
The program of SURROGATE maternity can be applied in different ways, in cases when the genetic mother cannot carry the fetus for any reasons, for instance with serious uterus trauma or absence of the uterus.
When female reproductive cells are required for solving the problem :(a little about donor programs)
Of over 300 thousand oocytes present in the ovaries of a mature woman throughout her life only a few are used. Others are ejected from the body together with the endometrium layer each lunar month.
Donors of oocytes used to be looked for among relatives of sterile patients, but today experts agreed in favor of the selection of professional donors. This significantly increases the results of sterility treatment and enables one to avoid numerous ethical problems.
Firstly, as a rule, women relatives do not possess those high reproductive abilities which are typical for the donor, chosen on a competitive base.
Secondly, a close female relative of the patient may also be the carrier of not completely expressed family genetic problems.
In addition, with the division of roles between the donor and the recipient (one woman gives egg cells and the other prepares to accept embryos into the uterus) the chances for success rise.
One woman - an anonymous donor - performs her noble and materially supported mission and the other woman - the recipient - does not get into an ambiguous situation with the biological, social and juridical status of the future child. The facts are known only to the doctor who carries out fertilization "in vitro". The child belongs only to the spouses who asked for help and became the initiators of this procedure for overcoming sterility. They carry all the parental duties towards their child.
Donor oocytes are used in cases of deficient function of ovaries including conditions of premature menopause, carrying of defective genes, disgenesis of gonads and so on. In addition, becoming pregnant for women over 40 is quite difficult but thanks to the application of donor oocytes completely normal children are born to women even after the menopause.
About surrogate maternity.Patients suffering from the inability to bear a child and even those with no uterus, could not even dream of having a genetic child of their own before. Today there are tried and tested methods of putting healthy children into the hands of happy parents.
Embryos are fetuses capable of development in the favorable conditions of the uterus of any healthy woman. The method of surrogate maternity is based on this phenomenon. It occurs when a child is carried by another woman. Sometimes a close relative of the patient or even her mother can be this woman. More often - it is a woman who becomes in effect a close friend and gets a big financial reward. The best option is cooperation with a woman for whom the state of pregnancy is natural and enjoyable. She can carry out her mission and help another woman, guided not only by financial stimulus, but doing something she enjoys.
Women oocyte donors and potential surrogate mothers undergo preliminary examination including gynaecological examination, ultrasound and hormone status. Those with latent infections and
other negative factors are excluded. Applicants must have children and be irreproachable morally and ethically. Modern genetic examinations of donors are also carried out: the chromosome set is examined and the carrying of the most widespread hereditary diseases is excluded.
In some countries (for example in Germany and Israel) the donation of female reproductive cells is not carried out, thus creating what is called reproductive tourism. Today hundreds of sterile couples from Europe arrive with hope in The Ukraine, which has always been the world`s great reserve of beauty. Many of them (almost one couple in three) take away something more than just good impressions.
